FAQ’s-
Fort Worth Functional Medicine is a fee for service practice and does not take insurance of any kind, including Medicare.
HSA/FSA accounts can be used for payment.

At the patient’s request a superbill can be provided for the visit, which the patient may present to their PRIVATE insurance carrier. Superbills cannot be submitted to Medicare for payment.

Dr. Jordan does not function as an acute care or primary care provider, and rather acts as an extension of your medical team.
It is strongly advised that you maintain a relationship with an outside physician to act as a primary care provider and to provide emergency and urgent care services.
